State police say a Brooklyn man died late Tuesday night after veering his minivan off the road in Monroe and crashing down an embankment.

The single-vehicle crash happened just after 11 p.m. on Larkin Drive.

The victim is identified as 62-year-old Ulugbek Yunussov.

Authorities say Yunussov was traveling westbound in a 2012 Chrysler minivan when he crashed. Police at the scene told News 12 that other motorists called 911 to report the victim trapped and unconscious and that rescuers had to break the windows of the vehicle to get him out.

Yunussov was taken to the hospital by ambulance where he was pronounced dead.

The crash remains under investigation.

Witnesses are asked to call state police at 845-344-5300 and reference case NY2400982330.
