The City of Yonkers is asking residents who had storm damage this past weekend to fill out a damage survey because they could be eligible for disaster assistance from the state.

The form is for anyone statewide whose business or home was damaged.
It may help state and local officials request federal assistance.
The survey is to gather information - residents should still report damage to their insurance agency.
