A 35-year-old woman has died after she was found with trauma to her head, face and body in Melrose, according to the NYPD.

Police say EMS responded to 370 E. 158th St. around 6:30 p.m. Sunday and took Jasmine Chisolm to Lincoln Hospital in critical condition.

Chisolm was pronounced dead Monday morning.

The medical examiner will determine her cause of death.

No arrests have been made, and the investigation is ongoing.
