A Spring Valley man operating a forklift was killed when boxes of tiles fell on him in West Haverstraw.
Police say 52-year-old Carlos Ramirez-Hernandez was found unconscious at KJ Tiles on Beach Road Thursday morning.
Paramedics pronounced him dead at the scene.
A preliminary investigation revealed that Ramirez-Hernandez was operating a forklift and reversed into commercial metal shelving containing pallets of tile.
The impact from that collision caused the tiles to fall on him.
He was pronounced dead at the scene.
While the KJ Tile business in Monsey is closed on Fridays, management did give News 12 a statement by email saying they are saddened by the loss. The statement called Ramirez-Hernandez a devoted employee and an amazing individual. They are grieving with his family.
The building has been slapped with an order to vacate because the town of Haverstraw Building Department deemed it to be unsafe.
Occupational Safety and Health Administration confirms it opened an inspection at the warehouse and has six months to complete it. Until that's over, it will not be able to comment on the incident.
