Army officials say human error is to blame for a New York National Guard helicopter crash in January that killed an Orange County man and two other soldiers.
Chief Warrant Officer 2 Daniel Prial, of Warwick, and the two crew members were killed when the Black Hawk helicopter went down during a training flight near Rochester.
In a statement, the National Guard says the crew was performing an emergency training maneuver when the procedural error occurred.
They said since the crash, the Army has restricted practicing that emergency procedure to flight simulators only.
The investigation includes a review of the aircraft itself, maintenance records and the flight recorder.
Families of the victims were notified of the findings in personal briefings.
