The National Transportation Safety Board is releasing information about a plane crash in Costa Rica nearly two years ago that killed 12 people, including a Scarsdale family of five.
According to findings by the NTSB, numerous factors contributed to the deadly crash.

Irene and Bruce Steinberg and their three sons were among the those killed when a Costa Rican tourist flight crashed soon after takeoff from a mountainous region.

The NTSB now says the flight crew failed to maintain airspeed when leaving the high-terrain area and that pilots could not determine the wind speed.
