NYACK - For the third time since September, a barge came loose from the Tappan Zee Bridge construction site Sunday afternoon.

The incident occurred around 4 p.m. There were no injuries.

In a statement, Brian Coneybeare, Gov. Andrew Cuomo's special advisor on the project wrote, "Safety is the Thruway Authority's No. 1 priority on this project. It is unacceptable for construction barges to come loose on the Hudson River.  The Thruway Authority is currently reviewing Tappan Zee Constructors’ safety protocols and barge mooring methods."

Coneybeare also said that the contractor has been directed to install GPS trackers on its barges, boats and other vessels as soon as possible.

In September, residents called police after a 50-foot barge broke loose from the site and drifted on the Hudson for nearly a mile. A pair of barges broke loose from the site in January.

In July, two people died when the boat they were riding in crashed into a barge near the construction site. Jojo John, the boat's operator, faces several felony charges as a result. His lawyers say the barge was not properly lit and John did not see it.
